Referral Process: Clients can self-refer by completing an intake form. Partnership referrals are processed according to the pathway established in the agreement using an agreed upon referral form or an intake form which includes the logo of the partner on the form to distinguish between forms.

Waitlist: To become a patient of the CHC, the waitlist is approximately 1 year long. However, there is no waitlist for HIV positive clients. Clients are informed of the length of the waitlist when they drop off their intake form and are offered resources and information on other services.

Wait Time: There is no wait time for HIV positive clients. HIV positive clients fill out an intake form, and the HIV Case Manager will contact them within 48 hours for an intake; depending on circumstances the client may be seen right away.
