Referral Process: Referrals are accepted in any format as long as there is a positive HIV test, and the referring physician information. Clinic will provide an appointment date and request any other information that would be important (VL, genotype, CD4, HIV notes). Referral form or letter can be faxed to 416-340-4890. Referalls sent through UHN would go through EPIC. Clients cannot self-refer.
