Referral Process: Patients should be within the Toronto Centre catchment area. Self-referrals are not accepted. Referrals should be emailed to Pegi at [email protected]. Pegi will then look for physicians within the primary care group (4 different clinics) who will take them on. This tends to be mostly a small handful of physicians at Wellesley St. Jamestown FHT site, who aim to take on one new HIV+ client per week in accordance with Positive Care Coordination.
