Please include the following information: Patient Demographics (name, DOB, contact info/address, HC# or type of insurance (IFH, Morecare, etc.) Referring Provider Info, incl. billing# Current HIV medications/amount left If available: Positive HIV test result Most recent CD4, HIV Viral Load Genotype HCV genotype and HCV RNA Baseline lab work, diagnostics Previous counsult note Referrals are accepted via fax (705-523-7077) or email ([email protected]).
