Referral Process: Process for referral: Referring provider can call the REACH Admin person at 905-730-5647 to set up a transfer. Clients can self-refer, which is best done by contacting our partner, Positive Living Niagara (905-984-8684, ext. 124 for the Care Coordination Supervisor), which has excellent case managers that can also help clients with any 'social determinants'. A specific referral form is not required, but referrals should include: reason for referral, client contact details, past medical history, allergies, list of meds, lab/imaging results for the past year, current pharmacy, current primary care provider, and case manager (if any).
